What is Ascension Care Management ACO?

Through a partnership with Ascension Wisconsin, Ascension Care Management’s Accountable Care Organization (ACO) serves Medicare Fee-for-Service beneficiaries. Ascension Care Management’s goal is to help Medicare beneficiaries successfully navigate a complex healthcare system, receiving the best possible care and support while moving toward better health.

What does Ascension Care Management mean for you?

Since you have Medicare Parts A and B, your existing services are not going to change. Ascension Care Management’s care team is cooperating with your doctor to provide new services at no charge, and will help you navigate a complex healthcare system.

Care Team: Here for you

Working with your provider, our care team will be glad to discuss your care, work with you to develop disease management and wellness goals, and connect you to other community resources. Ascension Care Management’s care team includes nurses, social workers and counselors who are trained healthcare navigation specialists. Your care team will help you find the facilities, care and services you need.

Ascension Care Management is not: A Supplemental Insurance Provider or a Medicare Advantage Plan
